Prescribing & Dispensing Alpha-Stim (US)
In the United States, Alpha-Stim devices are available by prescription and are FDA cleared for the treatment of anxiety, insomnia, and acute, chronic and post-traumatic pain. They can be prescribed by licensed healthcare practitioners both in-person in a clinical setting, as well as via telehealth.
To make prescribing as easy as possible, we use a Statement of Medical Necessity form. This functions as the script for the device and contains all information needed to dispense Alpha-Stim quickly and efficiently.

Download and complete the Statement of Medical Necessity Form as you would a prescription. It serves as the official order for Alpha-Stim. - Submit the Form
The completed form can be sent by email to info@epii.com or fax to (940) 328-0888. - Device Dispensing Options
- Provider Purchase: Order through a local Alpha-Stim sales representative or by calling 1-800-367-7246.
- Patient Purchase: Once a prescription or Statement of Medical Necessity has been obtained, patients can order online directly through the Alpha-Stim website.

Will health insurance cover Alpha-Stim for patients?
Insurance coverage varies by policy. Durable medical equipment coverage is often required to obtain some level of reimbursement for Alpha-Stim devices. EPI does not bill private carriers but will assist customers by providing a paid invoice and a sample Statement of Medical Necessity for your practitioner to complete. This statement is typically required by the insurance carrier if the device is covered.
The HCPCS code for the Alpha-Stim AID is E0732 and the Alpha-Stim M is E1399.
Patients can also use a flex spending account (FSA) or health savings account (HSA) and may be able to qualify for financing and flexible payments.
